Questionnaires¶

A questionnaire is a form prepared by the care provider to be filled by the patient. A questionnaire provides the user with a list of questions that:

• Provides feedback from the patient.
• Prepares a consult.
• Responses used for a basic symptom checker.
• And more.

The questionnaires are analyzed. Depending on the provided parameters, the answers can trigger alerts. These inform the SCP operators on the seriousness of the complaints if they could become an emergency, and more.

The type of answers to the questions in the questionnaire can vary:

• Multiple choice
• Free text
• Numerical values
